After a health plan processes a claim, the Explanation of Benefits (EOB) shows how the plan handled the billed services and what it says the patient may owe. The provider then sends the actual bill.
CMS advises comparing the patient-responsibility amount on the EOB with the provider bill. A mismatch is a reason to investigate before paying, not proof that either document is automatically correct.
- Date of service
- Claim number
- Provider/facility
- Drug and administration lines
- EOB patient responsibility
- Provider bill balance
- Network level
- Authorization reference
- Any appeal or reprocessing reference
Match the claim before comparing the dollars
Confirm the date of service, provider/facility and service line. An infusion visit can generate separate claims for the drug, administration, labs, facility charges or professional services.
A bill may combine multiple claims while the EOB separates them, so compare like with like.
If the provider bill is higher than the EOB patient responsibility
Call the provider billing office and give it the claim/EOB reference. Ask whether it has received the final adjudication and whether the account needs to be adjusted to the contracted patient responsibility.
Do not assume a balance is correct merely because the provider statement printed after the EOB.
If the EOB itself looks wrong
If the plan processed the wrong network level, failed to apply an authorization, used the wrong benefit, or denied a service you believe should be covered, the dispute may belong with the insurer first.
Ask for claim reprocessing or use the appeal rights in the EOB/denial. Keep the provider informed so the account does not move forward as an ordinary unpaid balance while the claim is under review.
Use the source that actually controls your case
MedicineCostHelp can organize the problem, but the member-specific plan, Medicare program, provider billing record or assistance program makes the actual decision. Save written determinations and reference numbers so the next person does not have to reconstruct the case from scratch.
